All states require each facility to have a licensed dental practitioner as the registered Radiation Executive Officer (REO) for each location. The REO may designate another licensed individual as the Radiation Safety Officer (RSO) to assume responsibilities for maintaining the Radiation Safety Program. The program questionnaire must be completed by either the licensed REO or a designated RSO that is familiar with the radiation generating equipment and clinical procedures of the facility.

Before you begin, we highly recommend having the following information ready and available as you will need it to complete your onboarding questionnaire:

• Certificate of Registration
  
• Facility Floorplan
  
• Report(s) of Assembly (Form FDA 2579)
• Device Calibration Report(s)/Evaluation(s)
  
• Radiation Expert/Physicist Evaluation(s) (if applicable)
  
• Most Recent Inspection Report(s)
